      ♦  All apparel questions should be sent to Steven Carrington
      ♦  N/C setups for apparel orders over 2K units, $10.00 setups per location/color otherwise
      ♦  Screen charges holding at $0.20 net per color/location per Josh
      ♦  Customer is not charged for samples so long as the per piece cost is less than $10 at EQP NET pricing
      ♦  $0.20 to both polybag & sticker on apparel items
      ♦  No repeat setup charges
      ♦  No PMS Match Charges
      ♦  4% Rebate on Domestic Goods Only
      ♦  Program Orders: Customer gets EQP and no LTM fees so long as they are ordering at least half of the first column
